Visa Rewards card was promised 10 months ago, still haven't received it

Hi when I signed up to Comcast in April of last year I was promised a $200 Visa Rewards card. The represenative said I would receive it after 3 months. It's now been 10 months and I haven't received it. I still have the paper the represenative gave me that has all of this written on it. I tried going to the xfinity rewards website but it says no records found.

In order to qualify for a Visa Rewards Card the following conditions must be met:
 
  • You must be a new customer
  • You must sign up for a qualifying bundle with no modifications
  • You must make full payment on time for 90 days (no late pays)
 
Please note that this card comes with an expiration date
 
You can check on the status of your card here after the above conditions are met.

Yes I know it says that in the FAQ which I already read. I should of met those qualifications in July. When I try to check my status, as I already said, it simply says "no records found".
