Poor signal, modem reboots, blocky tv with audio cutting out

Yeah, its summer time and the same problem I had last summer is back.  Extremely poor signal strength... modem shows 51db+ for upstream, -3db downstream and a lot of errors and reboots.

Multiple TV channels are blocky with choppy audio to the point of being unwatchable.

 

Had a tech come out last summer and checked signals at the street tap and said they were bad, put in a ticket and had something fixed somewhere up the line... but now the same problem is back.

How do I get a tech out here again?  It was extremely painful to go through all of the red tape and finally get somebody to see the problem.
