Pay per view that wasn't ordered

I have had problems with someone ordering pay per view and charging our account. Last year I set passwords and also blocked channels. The problem stopped. The last few weeks it has started again. When I went to the password it had been removed and channel blocks removed. I reset the password someone has again ordered and recorded something. No one in the house has the password other than me. What can I do?
