Hard Inquiry on credit report

5 year customer.  I was speaking to a representative about my account that was canceled.  My account was on seasonal because I was in the process of building a house.  Then out of the blue, my account, that I was paying for, was canceled.  I called to find out why but no explaination.  The rep said I cold sign up as a new customer and I told him I will call back once I move into my house.  Next thing you know, Comcast made an inquiry on my credit report that I did not authorize.

- Your full name, address, and a phone number associated with your account.
- The exact date of the inquiry on the credit report
- The name and address that listed on the credit report
- The screenshot of the report if possible.
