Deceased Account Owner - Need email to send affidavit

Deceased Account Owner - Need email to send affidavit

My father recently deceased and I cannot get through on the Customer Service line to get the email or fax number to send the affidavit with the death certificate. The hold times are 1.5 hours +. There not information on the website telling you where to send this, only a link to the actual affidavit that needs to be sent in. I cannot turn off the automatic payments either so they are due to bill my credit card on April 1st again. Please help.
