Being charged monthly for a modem I own

I turned in my modem at least 3 years ago and bought my own.  I noticed on my bill this month that I am being charged for a modem rental.

I went back through July 2018 (as far as my bills go back on the Comcast website) and I see I have been charged all this time!!  How do I go about getting a credit on my account?
