This is Barbara <Edited for violating forum guidelines: "Personal Information">.  I am contacting you to advise you that my husband, Andrew <Edited for violating forum guidelines: "Personal Information">, passed away on June 30, 2018.  I am requesting that the account be placed under my name.  How do I do this?
