$5 a month Ipad still has not shown up

I ordered the $5 a month Ipad and it still has yet to show up.  I have called and been passed around to Billing department, Internet department, and mobile department and still can not find out if this thing has shipped, been lost, ect... some help would be appreciated.

Been on order for over a month, $5 added to my bill but no Ipad or status update just responses repeating no update yet.
