Remove old equipment from our account

How do we get returned and old equipment from our account?

 

I returned dvr and an old smaller box to comcast.  The smaller box no longer shows on our account but the old dvr does.

We also have two modems showing on our account.  I don't see anywhere on the site where I can remove.

 

Please advise.
