ID theft/ Fraud form

Hello,

I have working to complete the Comcast fraudulent account form and have run into the following issues:

 

1) Step 2 of the form on the Comcast website requires that I colse all fradulent accounts and follow up in writing.  The snag I run into is that Comcast representatives on the phone routinley assure me that the account is closed, only for that to end up being untrue.  Whether they are being truthful or not though the form seems to require that I get something in writing from Comacst saying the account is closed... they refuse to do that both on the phone and in the store (where the people on the phone promised me I could get it).

 

2) I was more recently informed that I am filling out the wrong form and need to start over.  The person referenced something called the "simple form".  Neither I nor the people in the Comcast store could locate that form (although in fairness they didnt exactly try super hard).  Is there a secret form not on the website that I should be filling out or am I filling out the correct one?: https://cdn.comcast.com//cdn.comcast.com/~/Media/Files/fraudclaimform/Fraud-Packet.aspx

 

I just need to fill out the form so Comcast can be willing to help me... I am also filling a "Consumer assistance request" form with the State Attorney Generals Office to compel Comcast to honor their own process, but Idk how long that might take.

 

Should I sent in the form incomplete without written proof I closed the account?

That is the same form listed in the link above, so it seems you have the right form.

IMO, Yes. Note that Comcast requires the form to be notarized and a copy of a police report be attached and sent with the form, amognst other things. All requirements listed in the link at the top of the post. Good Luck.
