Help with bill from back in April 2017 moved and was charged 3 times for install

Hi I have been talking to agents since april to find out how to get these charges off my april and may bill in 2017 I have the pdf's of my bill and was charged 3 times for install 60.00 each time and nobody would help me with this. so the other day I finally got a rep to go back and she did see all the charges and agreeded they had charged me 3 times, but she couldn't remove them because they were past the 4 months mark, can someone please lead me to the right person so they can straighten out my bill for me please
