Being charged modem rental for equipment that I own

I was offered a special rate in July 2017 and since then I’ve been charged the modem rental fee for a modem that I own. My billing history shows that I wasn’t charged the fee until that point, even though my equipment never changed. I've talked on the phone with CSRs twice and once in online chat and they say the issue is resolved but it never is. Can someone help get this charge permanently removed, and get my account credited for the months I was incorrectly charged? Thanks.
