Being charged for equipment already returned

I returned all 3 boxes I had at the end of June.  Now on my July 15th bill I am being charged a total of $5 monthly(2.50 each) for two boxes even though I returned them way before this bill date.  I was concerned when I walked into the Fort Collins store with my 3 boxes and they told me to just set them down and go wait for an agent to talk to about my bill.  I watched about 6 other people also pile their equipment next to mine.  After seeing this charge I attempted to start a chat but it asked me for name, email, zip code and description of issue.  When I filled all this out it said I needed to enter a valid phone number even though there is no box to enter a phone number!! SO it wont even let me chat and I dont have 30 min right now to sit on the phone.
