Internet connection resets every few minutes

Okay I'm going to try to give as much info as I can so that someone can give me response soon..

 

For the past several months I've been having trouble with my internet connection. I seem to be able to use internet browser as long as it's just forums(no videos) and less than 2 tabs. Every time I open up multiple tabs and expecially when I play a game after a few minutes the cycle begins....

 

My internet shuts off for about 20-40sec and then works again. Once the internet works again I start to play a game and about 30 seconds-2 minutes later the internet shuts off again. When I look at the modem error log:

I have had a tech out 3 times and replaced my modem twice and router once. I have connected my computer/xbox/ps3 direct to the modem/router and used wirelessly and the problem still occurs. I am so tired of this problem and I really hope that someone can figure out what the heck is wrong so that comcast can come out and fix it.

What kind of modem is it  Model name and number

 

If you go into 192.168.100.1 you can look at the status page and it will show Downstream levels and upstream levels if it is a docsis 3.0 modem you will have up to 8 downstream and up to 4 upstream all the levels will help me troubleshoot

 

The numbers you have are hard to read since they are just codewords

The forward power is good. The forward SNR is borderline. The return power is borderline.
