DPC3941T Gateway ethernet ports not working

DPC3941T Gateway ethernet ports not working

One port works fine, but none of the other three work at all, no lights, no connectivity.  When I pull up the LAN Hardware profile on the router, it shows both ports 1 and 2 as "Active", and ports 3 and 4 as "Inactive", but only one port will actually function.  I have verified that the router is not in Bridge mode, and I am at a loss as to why I can only get one ethernet port to work at a time...

I had a perfectly functioning DPC3941T and Comcast notified me that they were sending me a replacement.  I received an Arris TG1682G, sent back the Cisco.  I got the Arris added to my account and haven't had any issues with it.
