Pixelization still after tech visits

Have had intermittent pixelization issues for about 6 months. Had a tech out and he couldn't find anything. Put new connectors outside and left me a replacement amplifier in case the issues came back. It only took a week to pop up again and I changed the amplifier which made no difference. So my pixelization issues continue.

Also, they billed me for installation of new equipment $60. I have since called 3 times and each  time I'm told I will be credited. It has been going on a month and still have no credit.

 

Any advice for either issue? I can't bring myself to call yet again and talk to someone reading a flowchart to me. I really think the issue is between the pole and the can in the backyard since that is the cable there was meant problems with in the past.
