Equipment change?? To access On Demand??

Got something in the mail a couple of weeks ago regarding equipment change, didn't keep it and now all of my TV's except one will not let me access On Demand.  I have HBO and Showtime.  Did anyone else get a notice about equipment changes?  I'm in Southeast Michigan.
