What is the Internet/voice Equipment rental for?

The rental fee would be for your modem/gateway.  The box should be placed in a place where it can be seen and has air flow around it.  You are only limited as to the length of the coax cable.

Hi Adventure42, the information that (geek46) provided is correct. Your internet/voice rental is for your modem, you don't have to rent a modem from us (you can purchase your own) but you will need a modem to have internet in your home. "The box should be placed in a place where it can be seen and has air flow around it.  You are only limited as to the length of the coax cable." This statement is also 100% correct.
