How do you determine what's Spam and what isn't?  I even get emails from Xfinity marked as Spam.

Spam filtering systems work on a basis of input from known sources of junk mail, along with security companies like Spamhouse that monitor sources of junk mail.  But it also depends to an extent on user input-------people marking messages as either Spam or Not Spam.  The messages from Comcast getting to the spam folder are what are called "false positives".  Legitimate emails can sometimes get dumped into the spam folder.  Mark those as not spam to help train the filters.  You should find these  of interest----------------------
