telephone service out of order

telephone service out of order

tv and internet working fine....telephone has no dial tone.  Recent message indicated that connections needed to be checked.  I did so but found not loose connections.  modem shows 6 blinking lights--one on left and five on the right side of two stationary lights.  Recent visit by field serviceman installed new cable from pole to house.  can you diagnose problem?

Can you plug a phone into the LH1 jack on the back of the modem to see if you get a dial tone there? that would rule out house wiring faults.
