PHONE NUMBER TRANSFER ISSUE:

I need to get new info to an Xfinity employee named Blake. Blake is attempting to "Port" a phone number from my old phone service (Windstream) with the incorrect account information. The Windstream information on my old Windstream account has changed due to the Windstream account being closed then reactivated in my name (because my wife was not present. The old account was in her name.) for "Porting" purposes only. How can I get Blake the account number he needs to complete the porting process?
