Being Billed & Paying for Returned Equipment

We are being billed and paying for a modem and TV box that were returned. Unfortunately, we do not have any proof of the return of the equipment and did not notice the charges on our bill until recently. This is equipment that was returned a while back. We simply would like for the charges to be removed from our bill.
