Erroneous Modem Rental Charge

Erroneous Modem Rental Charge

For the past few months I've been charged an Internet Equipment Rental fee despite the fact that I have always used my own modem.

 

I've contacted customer support 3 times and each time they assure me that it won't happen again, but of course it does. What needs to happen to get the charge removed permanently?
