"Service to Additional TV" - Listed twice on bill

"Service to Additional TV" - Listed twice on bill

Under the Additional services and equipment section, it states "Service to Additional TV - with TV Box" twice, for 9.95.

I have only one TV, with one box connected to it.

Why am I being double charged?
