My purchase confirmation?

I have two questions before doing eSignature. Unless these problems clear up, I can't sign my order form.

1-package,service, and equipment fees?

What is this charge? Is this an every month thing because it's more then the bundle I picked out.

If "package, service, and equipment fee" is charged every month, it would be double price. I will have to cancell "Internet Pro Plus with HBO" plan I have just ordered. Because only Internet & TV services are too expensive! It's incredible!

2-On My purchase confirmation, the figures do NOT add up. Is this miscalculated? Please explain me the detail of my bill as soon as possible.
