Charges for Unreturned Equipment

I recently cancelled my membership as I have moved and no longer require internet as it's already provided.

 

There are charges on my final bill for an unreturned cable card and an unreturned SD converter, both of which I never had.

 

I've also started to get calls from collection companies about a $120 charge for a cable box, which isn't even listed on my final bill, and which I also never had.

All of my internet equipment is my own, and I only ever had internet service, there should be no charges for any equipment period.

 

I've opened tickets about this problem over the phone twice yet I am continuing to receive calls every day from a collections agency.
